Latrobe Community Health Service (LCHS) provides planning and community connection services as a partner with the National Disability Insurance Agency.

LCHS Local Area Coordinators will be available at the library to:

• Connect Monash residents who are living with a disability to informal and natural supports that are already available within the community
• Provide assistance with the planning process
• Ensure plans are implemented effectively
• Assist with applications for the NDIS and answer questions.
