What you need to know

  • Participants must be registered with My Aged Care.
  • PAAC is run from the Oakleigh Community Centre on Tuesdays and Thursdays.
  • A fee does apply and covers daily program amenities such as morning tea and activities.
  • Participants are responsible for covering entry fees and food expenses on outings.

While members are encouraged to arrange their own transportation, it can be provided on request, subject to availability.

My Aged Care - how to apply

This social inclusion program is offered through the Commonwealth Home Support Programme (CHSP).

To access this program, individuals aged 65 or over can start by registering with My Aged Care and requesting a social group referral with City of Monash.

Contact My Aged Care on 1800 200 422 or visit myagedcare.gov.au to begin your journey.

After registering, My Aged Care will guide you through a series of questions to assess your specific needs. Subsequently, a referral will be forwarded to the Regional Assessment Service (RAS). RAS will schedule a visit with a Home Support Assessor. During this visit, you’ll have the opportunity to discuss your requirements and the services that best cater to your needs.
