Community Outreach assists Monash residents who may be having trouble accessing services and resources, ensuring they can feel connected and supported in the community.

Our Community Outreach Officer can:

  • Offer a safe and confidential space for older residents to talk about issues relevant to them
  • Provide information on a range of services and programs that can assist health and wellbeing
  • Assist with referrals to My Aged Care and other services and programs for support
  • Provide follow up support to ensure the resident feels connected and engaged with a service or program.

Translation services are available for residents from non-English speaking backgrounds.
